The safety of our clients and clinicians is our top priority
Well Life Counseling and Consulting, LLC is committed to the health and safety of our clients, community and clinicians. We have instituted procedures and policies for our clinicians to follow that ensure we continue to serve our clients, safely and conveniently. We are offering telehealth sessions using a HIPAA compliant, encrypted, professional platform, ZOOM business. Additionally, many of our clinicians have begun to return to the office and are conducting a limited number of in-person sessions. We are strictly following the CDC and State guidelines for reopening. If you come to our office, please abide by all posted notices. If you are currently a Telehealth client and you have scheduled an in-person session with your clinician, you will need to review, sign and return the document linked below. Additionally, this document can be easily sent to you for review and signature using our client portal.

For NEW CLIENTS that are utilizing TELEHEALTH for their first session, all of the pertinent information will be provided during your registration phone call. Registration Documents will then be shared with you via our Client Portal. For those clients that would like to complete the registration paperwork by hand or electronically, download the PDF linked below and contact the office.
